Your appointment time has been reserved just for you. As a courtesy to fellow clients and the provider’s time. CDN Eldercare requires a minimum of 24 hours notice for any cancellations or changes to your appointment. Please understand that our professionals only get paid when they deliver treatment. Client’s who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee, equivalent, to the rate of missed treatment. All clients are required to have a valid credit card on file and any outstanding fees will be applied to your account and charged. If you have an emergency, please let us know so that we can treat your specific situation with personal attention.

If you are unable to keep your appointment, for any reason, please notify us by phone / voicemail or email at your earliest convenience. If we do not hear from you, this policy will be applied.

Please note that the management and safekeeping of stamp cards and gift certificated are the sole responsibility of the client. Please be advised that the company does not assume responsibility for lost of misplaced stamp cards and gift certificates.

Payments for all treatments, whether private or insured, is ultimately the responsibility of the client.
